Send a safety report by email

Please include, if available

• Your name and contact details
• Your order number
• The product name, batch number and expiry date
• A description of the problem or side effect, and when it started
• The patient's age and sex (no full name needed)
• Any other medicines or supplements taken at the same time
• Whether a doctor or pharmacist has been consulted

All reports are handled confidentially, in line with data protection law. Please report even if you are not sure that the product caused the problem.
